Anime Club Permission

I am the parent and/or legal guardian of a child who wishes to participate in Anime Club,  between the ages of 11 and 18 years of age.

I understand that by submitting this permission slip, I am allowing my child to participate in the Library’s Anime Club. I understand that this means that at this club, my child may view TV shows or movies with a TV-G through TV-14 or MPAA rating of G through PG-13 and that these films or shows may include content reflective of those ratings.

No TV shows or movies with ratings above PG-13 or TV-14 will be part of club activities. If a TV show or movie is unrated, library staff will preview its contents and make a summary with their suggested age rating and content notes available for parents and guardians upon request.

Furthermore, I perpetually release CCPL and its employees from any and all liabilities that may arise from the consequences of my decision of allowing a minor to participate in the program.
